Whether a portion of dexterity has already returned to the hands or the journey is just beginning, hand rehabilitation exercises aim to reinforce strength and motor skills often weakened during a stroke.

Muscle spasticity

When a stroke occurs, the hands may experience increased tone or stiffness due to disrupted connections between the brain and the hand muscles, this is called spasticity. Without this strong neural connection, it may become more difficult to fully straighten the fingers or grasp an item.

As hands recover, strong signs of improvement include complete extension of the fingers without assistance. Simple, repetitive rehabilitation exercises strengthen the missed connections between the mind and muscles, adjusting the pathways and preventing the spasms. A series of treatments are often prescribed to relax, stretch, and strengthen the hand muscles.

Hand strength and function

The grip, strength, and overall function of the hands are often additionally impaired, making it difficult to completely lift, grasp, or release items. This complicates everyday tasks and possibly the ability to be independent with functional activities. Additional issues include loss of sensation—sometimes causing a pins-and-needles feeling—or periods of swelling. Intensity of pain and loss of sensations caused by the stroke are often dependent on whether the neurological damage occurred on the dominant or non-dominant side of the brain.

Once the stroke survivor comes to Steadfast Neuro Super Specialty Rehab Centre In Bangalore, India, we start with intensive stroke rehabilitation which includes physiotherapy, occupational therapy, cognitive behavioral therapy, manual therapy, constraint induced movement therapy, motor relearning program (MRP), proprioceptive neuro muscular facilitation (PNF), and dry needling.

Hand rehabilitation is the part of intensive stroke rehabilitation. The stroke survivor receives many intensive functional training programs like hand extensor Bobath therapy, finger Bobath therapy, elbow Bobath therapy, shoulder Bobath therapy, ball grip, ball b/w thumb & 2 fingers (extended), pinch, thumb opposition, fingers side squeeze, fingers scissor spread (rubber band), thumb extension with rubber band, thumb pinch strengthening, thumb adduction, finger hook with putty, full grip, finger pinch, finger extension, bottle drop, ball drop, pinch release, pen spin, coin pick up and drop, behind the waist – cup pass, external rotation with canes, shoulder shrug in front of mirror, towel slide on bench in all directions, ball pass front to back (circle pass), moving ball in different postures, holding bal in standing, towel slide on bench, finger walk up & down, catching & throwing the balls.

The stroke survivor also receives training on eating, drinking, bathing, cleaning, cutting vegetables, cooking along with dry needling for entire spastic group of muscles, manual therapy,  PNF exercises, hand sway while walking, strengthening, upper limb relaxation exercises, weight shift, balance exercises and myo fascial release for entire upper limb (hand).

Physiotherapy, occupational therapy & cognitive behavioral therapy may  extend up to 4 to 6 hours a day for many months.
